Adhikari was the first Communist Prime Minister of Nepal. He died on April 26, 1999 at the age of 78 following a heart attack in the course of the election publicity campaign.
He was appointed the Chair of CPN (UML) formed following the unification between the then CPN (ML) and CPN (Marxist) in 2047 BS. As no political parties could secure a clear majority during the 1994 mid-term election, he as the Chair of the largest party became the 31st Prime Minister of Nepal (1994-1995).
